Am I Eligible for CDPAP?

Do you need personal care and assistance with daily tasks, in your own home? Are you eligible to enroll in a Medicaid Managed Long Term Care (MLTC) plan? Can you or your designated representative choose, train, and supervise a caregiver? If you answered yes to each of these questions, you are likely eligible!

Whom Can I Hire to Be My Personal Assistant (Caregiver)?

You can choose an adult child or other family member, a friend, a neighbor, or even a personal aide you already know. You can’t hire your spouse or your designated representative. Parents also can’t be hired as Personal Assistants for a child who is under 21 years of age. 

How Many Personal Assistants Can I Have?

You can have just one Personal Assistant, or you can hire more than one to work at different times as long as you stay within your allotted weekly hours. Your MLTC plan determines the number of hours per week that you’re eligible for care.

How Much Does CDPAP Cost?

CDPAP is free! The caregivers you hire as your Personal Assistants will be paid by Community Home Care through Medicaid, at no cost to you. You must receive Medicaid and be enrolled in a MLTC plan to enroll in CDPAP.

How Do I Enroll in CDPAP?

Give us a call! It’s our job to make the process quick and simple. We will guide you through it step by step, come to your home to meet you and the caregiver you choose, and help you complete the paperwork required.

What Does Community Home Care Do?

We’re a Fiscal Intermediary (FI) for CDPAP. Medicaid requires CDPAP recipients to work with an FI to participate in the program. We help you and your caregiver enroll in CDPAP, manage employment details, pay and provide benefits to your Personal Assistant, serve as a liaison between you and your Medicaid MLTC plan, and more!

What Does Community Home Care Charge for Services?

Nothing! Our clients and their Personal Assistants are not charged for any of the assistance or services we provide.

Someone I Know Needs Care. How Do I Become Their Personal Assistant?

If someone you know wants to hire you as their Personal Assistant, have them contact Community Home Care at 845-444-2555. We will determine whether the person is eligible for CDPAP, then guide both of you through the enrollment process.

Do I Need Any Training or Certifications to Be a Personal Assistant?

No, you do not need any certifications or prior training. You just need to be a caring and compassionate person over the age of 18.

How many hours am I eligible to receive services for?

Your MLTC plan determines the number of hours per week for which you are eligible.
