CDPAP Puts You In Control

The Consumer Directed Personal Assistance Program (CDPAP) is a New York State Department of Health Medicaid program that lets you control your home care. You can choose, train, and hire someone you know and trust to be your Personal Assistant and provide the exceptional care you deserve. Your Personal Assistant will be paid through the CDPAP program.

The process is simple.

How to Qualify

We help you throughout the process of determining your eligibility for the CDPAP program.

We explain each step and help you submit paperwork to become eligible for Medicaid, home care, and CDPAP.

Enroll

Once you have selected a Personal Assistant to hire, we meet you both and guide you through the enrollment process.

We ensure that your MLTC issues the proper authorization documents for the number of hours approved.

Manage

You manage your own care by training, scheduling, and supervising your Personal Assistant. You decide if you want to change or add Personal Assistants.

We manage the paperwork. We make sure your Personal Assistant is paid on time and receives available benefits as well as make any Personal Assistant changes you request.

Your Home, Your Priorities

CDPAP gives you control over the personal assistance you receive in your own home. You’ll work with your health plan to determine what services you need from a Personal Assistant. Then you’ll select a Personal Assistant—a family member or friend—and train them to be your caregiver.

 

You create a home care schedule that suits your needs and lifestyle, choosing either a set schedule or a flexible schedule that changes daily.

Whom can you hire as a Personal Assistant with CDPAP?

You can choose a family member, a friend, a neighbor, or someone else you know and trust to give you the assistance you need. You can even hire more than one person to be your Personal Assistant. Your caregiver doesn’t need any special certifications to be paid as your Personal Assistant. You train and supervise your own Personal Assistant to provide the help and care you need.

 

Spouses, designated representatives, power of attorney, and parents of a consumer under the age of 21 are not eligible to be your Personal Assistant. However, other family members who live with you may be eligible.

What Kinds of Care Does CDPAP Cover?

Your PA can be paid to assist you with activities of daily living (ADLs) in the least restrictive environment (LRE) of your own home.
